For many years people felt that they were trapped between a traditional indemnity health insurance plan (a wide range of choice and high degree of security in the event of serious accident or illness which came at a high cost) and a managed care plan (a focus on preventative medicine at relatively low cost but with severely limited choice).

Today however it is possible to some extent to enjoy the benefits of both traditional indemnity insurance and managed health care through a variation on the original Health Maintenance Organization (HMO) model known as a Preferred Provider Organization (PPO).

A PPO is essentially an HMO which means that the insurance company will establish a network of healthcare providers and, in exchange for a relatively low cost, will encourage, or in some cases require, policyholders to seek treatment within the HMO's network. Where treatment is taken outside of the HMO's network much, if not all, of the cost of such treatment normally has to be borne by the policyholder. However, in the case of a PPO, the rules for policyholders who wish to seek care outside of the HMO's network are relaxed.

Within an HMO a policyholder is assigned to a particular doctor or primary care physician (often referred to as a gatekeeper) and the policyholder must go through the primary care physician in order to receive treatment. If, for example, the policyholder wishes to see a specialist then he or she will have to be referred by the primary care physician and may or may not have a say in which particular specialist they are referred to.

In a PPO however no primary care physician is assigned and so no referral is required. Policyholders are free therefore should they choose to do so to seek treatment through a specialist who is not a member of the HMO's network.

There are of course cost implications to this choice and policyholders will almost certainly have to pay more for treatment with a doctor or in a facility that is outside of the HMO's network than they would if they sought treatment within the network. Nevertheless, unlike the HMO model, the PPO gives the policyholder the choice.

The aim of this essay is to discuss and compare two health care systems in the USA, the GPs and NHS systems, the relation between expenditure on these systems and efficiency of them. Another aim of the essay is to compare heath care systems in different countries and find out whether the dominance of public or private spending is optimal for the society; here the comparison between UK and USA health care system will be appropriate.

In the USA nowadays the Private Health Insurance system is the major provider of health care cervices. Is this situation adequate to the demands of the tax payers and is it effective for different categories of people who need medical treatment? Should the ratio between private and public sectors in health care be changed and should the organization of both systems be reformed?

Let us first of all define what is health. These can be two ways to define it:
1)Negative: the absence of disease
2)Positive: a state of complete physical, mental and social well-being. (World Health Organization, 1946).

Therefore we can outline two aims of health policy: to improve health and to reduce health inequalities. In terms of the first definition, the US health care system is efficient. In terms of the world-accepted definition, the second one, the US health care system is expensive and in many cases not efficient. Let us analyze the relation between costs and effectiveness in this system.

First of all let us trace the development and progress of both health care systems. The basis for current health care system was established after the World war II, when private doctors were serving most of the market. The traditional perception of health care as one of the paid services was and is still remaining the leading one. But it doesnt give the poor, elderly and disabled people, who most often need the medical treatment, the possibility to receive this treatment. In 1965, however, the Congress introduced Medicare and Medicaid programs, which were tax-funded and offered medical help for those groups of society who were not able to afford to pay for medical care. Naturally, this has caused the growth of health expenditures.

In the 1970s the government was trying to stop the growth of spending on health care and offering different measures: price controlling and regulation of medical practice. These measures, however, didnt work, as the doctors have been protecting their incomes. We can make a conclusion that the concurrence solely between GPs, private doctors etc. is not enough to maintain the balance of prices and provide the needs of consumers. In 1980s a lot of insurance companies called Health Management Organizations (HMOs) emerged; they were not offering direct medical treatment, but were instead making contracts with private doctors and taking the insurance payment from citizens. This helped a little to reduce the growth of budget spending for medicine, but did not help to solve the problem in general, as they didnt manage to perform a constructive structural change in health care system. The NHS services remained necessary for the less protected groups of society and their financing still was growing.

The policy of both presidential candidates in 1990s included the concept of managed care, which meant the further development of HMOs network and the participation of the employers in it. But statistics in the beginning of 2000s has shown us that not very many employers have accepted this offer, and those who were using managed care, were mostly big corporations. In 2002 Maxwell and Temin published a survey of more than 500 corporations in the US and it turned out that less than 10% of the employers offered choice or fixed dollar contribution [7, p. 39 to their workers.

The reforms in 1990s years have been aimed to the reduction of budget costs involved into health care and stimulation of the concurrence between private doctors and NHS. In fact, they have contributed to the development of the private practice and GPs in the US and have strengthened their positions against those of the NHS workers.

Currently, the health care system funding in the USA is one of the highest in the whole world, and the ratio between GDP and health care expenditures is likely to become even higher in coming years. But there is a contradiction between these high costs and the effectiveness of the treatment provided by the system.

The reasons of this phenomenon are different: starting from the bureaucracy and great administrative spending in the NHS system, the lack of regulation of private practicing doctors, ineffective management system of public medical treatment, the tradition of waiting lists etc., ending with high tax rates and opacity of the NHS financing, and difference in NHS and GPs rates of earnings.

When working as an assessor in social care two of the values we apply in the assessment process are fairness and consistency. However errors and subjective views can and do occur at any stage of the process.

When assessing the candidates competence, as they work through the Health and Social Care NVQ or SVQ, it is important to be conscious of the common types of error so that you can guard against committing one of these errors yourself.

If you are new to assessing you could also feel a pressure to decide every candidate you assess is competent otherwise it reflects badly on you. This is not the case. We need to have good practice in health and social care services. There is a certain standard of competence that has to be reached. It is in applying this standard that the fairness and consistency are so important.

Some of the most common errors are as follows.

First Impressions

This involves a Social Care NVQ or SVQ assessor taking a liking to a candidate (or the opposite) and on the basis of this first or early contact viewing the candidates performance more or less favourably than should be the case.

If you imagine a candidate who is warm and welcoming when you first meet him/her you can also imagine how difficult it is to be objective, especially if being objective means perhaps jeopardising that warmth.

Halo/Horns Effect

The Halo effect involves assessors inferring good social care practice on the part of a candidate on the basis of previous good performance by that candidate, without him/her actually being required to demonstrate the performance to the current standard. The previous performance may not have been associated with the criteria now being assessed.

The Horns effect is similar to the halo effect, only the opposite. On the basis of previous not-competent performance by the candidate an expectation exists that future performances will also be not-competent.

Instead of waiting for the performance, the assessor infers (probably wrongly) that these future performances will be not-competent.

Similar to Me

This involves judging a candidate favourably because they carry out a piece of work like we would or have values which are just like ours. Ours may be the wrong way or not the required way!! It is also important to remember that there are often many ways of carrying out a task. Just because a candidate does something in a different way doesnt mean they are not competent

Stereotyping

Stereotyping is always dangerous and directly contravenes the professional value base. In relation to assessment, stereotyping can occur in terms of assuming a certain level of competence (or lack of competence) based on an apparent characteristic of a candidate. An example would be an expectation that a young person might not be expected to be competent in assisting a bereaved person, on the grounds that they dont have sufficient experience of life.

It is not only the assumption itself which is dangerous, but also the way in which it might lead an assessor to look for certain pieces of evidence.

Remember: whenever we assume it makes an ASS out of U & ME

Contrast Effects

This arises when one candidates performance is compared to that of another candidate by an assessor. The inferior performance may then be deemed not competent, no matter how it stands compared to the performance criteria and evidence requirements. Social care NVQ and SVQ Candidates must be assessed against criteria, not against each other.

Experimenter Effect

If youre not normally around the candidate whilst they are working, then your observing him/her will possibly intimidate the candidate your presence influences the performance. We all do strange things when we are intimidated! This may also have an effect on the way others involved in the observation (eg: service users, colleagues of the candidate) act. This can also affect the candidates performance. One way to check that the experimenter effect hasnt been an issue is to ask the candidate after an observation Did anything surprise you?

These are just some of the errors that can arise in the assessment process. The costs of a medical care provider who is not in your HMOs network won't be covered by the HMO. Also, in most cases the HMO network is small in size, so getting appointment with a PCP can be difficult.

A Preferred Provider Organization or PPO is very much like an HMO. The only difference is that with a PPO plan, you don't need to be examined by a PCP first. Although not required, you are advised to select a medical care provider from the PPO's approved list. If you should ever need to go to a specialist, you don't need to have a referral. If you visit a medical care provider outside your PPO's network, the co-payment is higher and the PPO part of medical care will be less.

PPOs do offer more amount of freedom but the costs involved are higher. These costs can be exceptionally high when you seek the services of a medical care provider outside the network of PPO.

A POS managed care plan is mix of HMO and PPO. It offers more freedom to choose a medical care provider and has lower costs. You can choose a PCP, but it is difficult to get a referral to a specialist. If you stick to the network, the paperwork is negligible and your co-payment amounts will also be at aminimum. A POS plan has no deductibles. This might make a POS plan appear lucrative, but POS plans are not very popular.

One business owner asked me recently, what he can do to lower his rising health care costs and I jokingly replied, Make your employees run laps around your building. The owner responded by saying, How many? That is just one example of the desperate measures small business owners are willing to take to get those health care costs in check.

Not a week goes by where there isn't some article related to the rising health care costs. I don't know the stats off hand, but I'm sure you have read how much of a percentage they have risen year after year. This week I read about how bosses are leading fitness classes at work to get their employees motivated to get in shape. With all the wild solutions suggested for this ongoing problem, one of the few acceptable answers seems to be exercise and wellness. Here are some simple ideas that you, a small business owner, can start to implement to help get those premiums in check. I wasn't too far off with my running laps idea. A lot of businesses will chart out a walk path around or near the office for employees to utilize. They can use the path after work or during the lunch hour and usually the path is designated with markers to indicate distances on the path i.e. mile, 1 mile, etc. It is a great way to show the employees that you, as the small business, care about their health.

Another easy way to help with the employees' overall health is to try and control the nutrition that is around the office. I don't mean telling the employees what to eat, but if you have vending machines, put some healthy choices in those machines. Why put those energy sucking foods in the machines (like chips, cookies, etc.) when you can put choices in there that will help stoke their energy such as fruit. Finally, I would assign an employee the position of wellness coordinator. Let that person be in charge of researching the simple things an employer can do to make for a healthier environment. They can help plan activities to coincide with months dedicated to health, like National Cancer Awareness Week or something like that. They can also help organize a Health Fair for the company. This is a great way to have other health and fitness vendors inform your employees of the different options out there to make them healthier.

The bottom line is that a small business CAN lower the health care premiums through exercise and nutrition, but it will take time and a consistent effort. Getting guidance from a wellness consultant could also be a great way to save money down the road. Ultimately, though if you choose not to go down the exercise path, you should play the lottery. You're going to need the extra money to help your business as your premiums keep rising and rising.
